How does typical anti-psychotics work in schizophrenia ?

A

They inhibit D2 receptors in both the mesolimbic and mesocortical pathway. Inhibition of mesolimbic pathway dopamine receptors alleviates positive symptoms. Whereas inhibition of D2 receptors in mesocortical pathway worsen the negative symptoms.

  • Inhibition of tublo-infendibular pathway leading to hyperprolactenemia and related symptoms.
  • EPS due to inhibition of nigrostriatal dopamine receptors. ( hours to days)
  • Occulogyric crisis (hours to days)
  • Akathesia ( days to months)
  • Psuedoparkinsonism ( Days to months)
  • Tardive dyskinesia ( Months to years)

What is the key clinical differentiator of serotonin syndrome from neuroleptic malignant syndrome ?

A

In serotonin syndrome there is hyper reflexia and dilated pupils. Whereas in neuroleptic malignant syndrome there is hyporeflexia and normal pupils.
