Organism of typhoid?
A
Salmonella typhi also called enteric fever
Feco-oral route
Contaminated food milk water
Houseflies are important vector

Pathogensis?
A
Bacilli localized in payer patches within 60cm, first inflamed ulcerated then perforate.

Clinical features?
A
Occur in step wise 3 weeks:
1 week: step ladder fever, headache, sore throat, bodyache, malaise, anorexia, tongue coated with rawtips
Bronchitis, relative bradycardia
In adult usually constipated but in children vomiting diarrha
2nd week: fever high, spleenamegly tender, diarrhoea dominant, rose spot rash on upper abdomen and back
3rd week: resolve or perforated if not treated well

A
Wbc: relative lecuopenia Blood culture most sensitive test Stool culture Bone marriw culture Pcr Typhidot now obsoleted Widal test bhi

Treatment?
A
Bed rest Hydrated Ciprofloxacin 750mg twice a daily for 5-10 days if not suitable then azithromycin For carrier: cipro for 4 weeks Cholecystectomy needed
