definition
A
relatively common condition characterised by abnormal flexion of the digits, caused by a disparity between the flexor tendon and the pulley that it passes through (i.e. the flexor tendon cannot pass through the pulley smoothly) most commonly affects the A1 pulley

presentation
A
- initially causes a painful clicking sensation as the nodule catches on and passes underneath the pulley
- eventually the finger will lock in a flexed position as the nodule passes under the pulley but can’t go back -through on extension
- a nodule may be palpated at the base of the affected finger

patients usually have to
A
forcibly manipulate their finger to regain extension

which fingers are most commonly affected
A
the middle and ring finger

management
A
- up to 3 steroid injections around the tendon sheath
- in persistent cases surgical division of the A1 pulley
