impingement syndrome is also known as
painful arc
defintion
syndrome where the the supraspinatus tendon is compressed in the tight subacromial space during movement producing pain
painful arc definition
pain between around 60-120 degrees of abduction as the inflamed area of the supraspinatus tendon passes through the subacromial space
causes of impingement syndrome
- tendonitis
- subacromial bursitis
- acromioclavicular osteoarthritis with inferior osteophyte
- a hooked acromion
where does the pain in impingement syndrome classically radiate
to the deltoid muscles and upper arm, tenderness may be felt below the lateral edge of the acromion
test for impingement syndrome
- HAWKINS-KENNEDY test where internally rotating the flexed shoulder causes pain
- jobes test
- scarf test
1st line management
conservative with NSAIDS, physiotherapy and analgesia and subacromial injection of steroids
how many subacromial injections of steroids can be used
up to 3
2nd line management
subacromial decompression surgery reading more space for the supraspinatus tendon to pass through
