What is resting tremor and what is a cause?
A tremor that occurs when the muscles are relaxed.
It is abolished by movement. Made worse by distraction.
Causes = Parkinson’s disease.
What is a postural tremor?
Key feature?
A tremor that is absent at rest and is present on maintained posture such as holding the arms out.
May persist during movement but key feature is that it DOES NOT get worse.
Causes of intention tremor?
Anxiety
Alcohol withdrawal
Benign essential tremor
Thyrotoxicosis (bilateral, symmetrical, and fast)
Drugs (B agonists = salbutamol), caffeine, lithium
What type of tremor is benign essential tremor? How is it inherited?
It is an autosomal dominant condition.
Causes a bilateral, symmetrical and fast tremor.
Symptoms IMPROVE with alcohol consumption.
What is an intention tremor?
What are the causes?
How do you assess it?
A tremor that is task oriented and made worse throughout range of movement.
Cerebellar disease (MS, stroke, haemorrhage) Wilson’s disease
Assessed by asking patient to touch their nose and then your finger.
Management of tremor?
If younger, always check TFTs and copper-ceruloplasmin.
1st line: treat underlying cause and can give some B blockers.
Dysfunction or embarrassment with essential tremor = 1) Propanolol 2) deep brain stimulation
Management of acute dystonia?
Procylidine.
