Sickle cell Active chronic graft-vs-host disease Coeliac disease Splenectomy may be indicated in: Thrombocytopenia Severe haemolytic anaemia; spherocytosis Rupture Hypersplenism with splenomegaly
What bacteria are those with a dysfunctional spleen most at risk from?
Encapsulated bacteria:
Streptococcus pneumoniae
Meningococcus
Haemophilus influenzae B
What will the blood film show in asplenic patients?
Howell-Jolly Bodies Target Cells Lymphocytosis Neutrophilia Decreased production of IgG and IgM Reactive thrombocytosis
When should immunization occur in a splenectomy?
If elective; at least 2 (ideally 4-6) weeks prior to surgery
Prophylactic antibiotics post surgery
Emergency: immunize at least 2 weeks post surgery and when well
Prophylactic antibiotics to be started immediately
What vaccinations are indicated in those with hyposplenism?
Hib/MenC (haemophilus influenzae B and meningococcus C)
PPV (pneumococcal)
MenACWY (meningococcus ACWY)
What antibiotics are recommended for prophylaxis in hyposplenism?
Amoxicillin
