Approach to investigation of lymphadenopathy and diagnosing lymphoma Flashcards
What can cause lymphadenopathy?
Lymphoma
Infection; viral (regional) or bacteria (generalised)
Metastatic cancer; breast, ovarian
Connective tissue dx; sarcoid, SLE
What can cause night sweats?
Lymphoma
infection
Menopause
Too thick a duvet
What can cause wt loss with lymphadenopathy?
Lymphoma
Other malignancy
Infections
What are some specific symptoms of lymphoma?
Itch without rash
Alcohol induced pain
Fatigue
Describe the lymphadenopathy in a viral infection
Tender Hard Smooth No skin inflammation Not tethered
Describe the lymphadenopathy seen in a bacterial infection
Tender HArd Smooth Skin inflamed Yes/no to tethered to skin
Describe the lymphadenopathy seen in lymphoma
Not tender Rubbery/ soft Smooth surface No skin inflammation Not tethered to skin
Describe the lymphadenopathy seen in metastatic disease
Not tender Hard Irregular surface No skin inflammation Tethered to skin
What should occur if lymphoma is suspected?
Biopsy
FNA or core biopsy is often insufficient
How is lymph node pathology assessed?
Histoloy; microscopic appearances Immunohistochemistry solid node Immunophenotyping blood/ marrow Genetic analysis Molecular analysis
What is the purpose of immunohistochemistry?
Confirming it is a lymphoma
Helps to subclassify
Looks at patterns of proteins on surface of lymphoma cells
Uses antibodies against these, and enzyme reaction
Brown = positive
What can be seen on histology of hodgkin’s disease?
Nodular sclerosing lesions
What CD can be seen in follicular NHL?
CD20 +ve
What CD can be seen in hodgkin’s disease?
CD30 +ve
What is immunophenotyping?
Cells tagged with antibodies attached to a flurochrome-emits a specific colour of light when a laser is shone on it
Determines patter of CD numbers
