What is the primary curative procedure for vulval cancer and what is the procedure?
Surgery
Local excision if small or a total vulvectomy +/- lymph node dissection
When may radiotherapy be used in vulval cancer?
Before or after surgery or palliative care
When is chemo used in vulval cancer?
Rarely, only in advanced cancer.
Cisplatin
What is the primary treatment for cervical and what dicates procedure?
Surgery
1a (micro) then tracelectomy to preserve fertility
Over 1b then hysterectomy. total keeps the parametrium (2a) or radical removes (Over 2b)
When is radiotherapy used in cervical cancer?
Pre or post op
What is the role of chemo in cervical cancer?
None
What is the primary treatment method and what are the guidelines for endometrial
As it is normally found early hysterectomy +/- salpingoophrectomy +/- lymph node clearance is usually enough
When is radiotherapy used in endometrial cancer?
Sometimes after surgery
When is chemo used in endometrial cancer?
Rarely and only if advanced - platin
What is usually the first line treatment for ovarian cancer?
Surgery
Debulking to remove visible tumour. hysterectomy +/- salpingooophrectomy +/- LN clearance +/- bowel deposits
Due to late presentation this is not normally curative
When is radiotherapy used for ovarian?
Rarely - only palliative
What chemo is used?
Paclitxel plus platinum
If it reoccurs within a year it is resistant so another is used. If after a year the same one is used
