post concussive syndrome presentation
headache, confusion, amnesia, difficulty concentrating and multitasking and vertigo, mood alteration, sleep disturbance and anxiety
When do symptoms of post concussive syndrome resolve?
resolve within a few weeks to months following TBI (can have symptoms up to 6 months)
What is post concussive syndrome?
follows mild traumatic brain injury and symptoms will improve within 3 months (70-80%) with targeted pharmacological and non pharmacological strategies.
When to get neuropsychological testing done in post TBI?
if it was moderate to severe TBI and not improving in expected time frame
When to get a EEG in post TBI?
used to rule out non convulsive subclinical status epilepticus. Used in moderate to severe TBI seen with brain edema or hemorrhage who were in a medically induced coma or having fluctuating levels of consciousness or frequent intermittent seizures without return to normal mental status.
Can someone develop depression with TBI?
can have comorbid psychiatric conditions like PTSD and depression that develops after TBI and can use antidepressants and psychotherapy to treat.
sports related concussion
they generally do not involve loss of consciousness and may be underrecognized.
symptoms of confusion amnesia and headache and dizziness and nausea/vomiting immediately following injury
sports related concussion
within hours to days may have changes in mood or cognition
do we ever see seizures in sports related concussions
yes if severe or complicated (by ICH) concussions
what happens if athletes with concussion return to play while symptomatic?
have higher risk for recurrent concussion and cumulative brain injury.
how do athletes return to play after a sports related concussion?
they need a 6 day graduated return to play protocol in which they are evaluated for recurrence of symptoms while performing stepwise increases in physical activity.
if they remain asymptomatic in protocol, they may return to play
is there a time frame that athletes have to remain out of playing after a concussion?
no set time frame. if symptoms have resolved then should do a “return to play” protocol to test and see if he can tolerate it.
athletes with recurrent concussion and persistent neurobehavioral abnormalities should
retire from contact sports
people who have persistent symptoms post TBI or concussion should get
assessments in cognition, blaance and neuropsychological testing as a part of comprehensive non traumatic brain testing.
