Independent transfer
No assist (verbal, manual, or device)
Modified independent transfer
Pt may require VC or assistive devices
Assisted transfer
Pt requires assistance from another person to perform the activity safely in an acceptable time frame
- physical, VC or TC
Standby assist transfer
Pt requires VC or TC, directions, or instructions from another person positioned close to, but not touching, the pt to perform the activity safely and in an acceptable time frame
- assistant may provide protection if safety threatened
Contact guarding transfer
Caregiver is positioned close to the pt with hand on the pt or a gait belt
- very likely the pt will require protection during the performance of the activity
Min assistance transfer
Pt performs 75+% of activity
- assistance required to complete activity
Mod assistance transfer
Pt performs 50-74% of activity
- assistance required to complete activity
Max assistance transfer
Pt performs 25-49% of activity
- assistance required to complete activity
Dependent transfer
Pt requires total physical assistance from one or more persons to accomplish the activity safely
- special equipment or devices may be used
