Non weight bearing (NWB)
Weight bearing as tolerated (WBAT)
Toe touch weight bearing (TTWB)
Partial weight bearing (PWB)
NWB
Non weight bearing
Cannot put any weight through that joint/extremity
WBAT
Weight bearing as tolerated
Put as much weight through the joint/extremity that they can tolerate with regards to pain
TTWB
Toe touch weight bearing
Carefully rest their toes on the ground but minimal pressure through extremity
PWB
Partial weight bearing
Put partial weight through joint/extremity
Typically noted as a percentage
Focus of exam
Screening needed to determine how to transfer or ambulate with a patient
Decide level of assistance, equipment needs, number of people needed to assist in moving the patient
Safe positions
Supine or in locked wheelchair with all parts attached
Body mechanics for sit to & from stand
Head - hips principle
During transfer, which side should you transfer patient to
Strongest side
Where does therapist stand for ortho patient with WB restriction on 1 leg
By strong/uninvolved LE
Where does therapist stand for neuro patient with weakness in 1 LE
By weak/involved LE
Safety procedure during transfers
Gait belt always placed before
Position wheelchair as close to surface as possible
Transfer to even surface - if not, higher to lower is better
Always transfer to strong side
Active transfer
Patient helps with transfer in any way
Passive transfer
Patient unable to help
Requires 100% assistance
