Definition of Tracheostomy
surgical opening of the trachea
Purpose:
to provide long-term airway support
BASIC PARTS OF TRACHEOSTOMY DEVICES
- Outer Cannula
- Inner Cannula
- Obturator
- Flange
Outer Cannula
- serves as an artificial airway
- directly inserted into the inner cannula
Inner Cannula
- serves as an artificial airway
- directly inserted in the outer cannula
Obturator
- serves as a guide wire
- directly inserted into both cannulas in the respi tract
- can correct dislodgement
- MUST BE AT BEDSIDE ALWAYS!
Flange
- located at distal portion of the cannula
- secured with tape & ties

Provide Privacy

Position: SF - to promote lung expansion
Nsg Con #3
wear sterile gloves

- open container kit
Container #1: 1/2 strength hydrogen peroxide
Container #2: PNSS

- Remove only the inner cannula using obturator - MANUAL
- Removed easily if cannula is automatic - COUNTER CLOCKWISE
- Soak w/ 1/2 strength hydrogen peroxide & clean with PNSS

Remove soiled dressing located behind flange

Wear another pair of gloves

Suction the outer cannula

Insert inner cannula using obturator again in CLOCKWISE MOTION

Clean flange & site using sterile gauze soak with PNSS
2 sterile gloves: 1 for flange & 1 for site

Apply again sterile dressing: size 4x4 / V-shaped

Chang the ties - apply/secure new ties and remove old ties
- square knot
- left over right
FEEDING PT.
Unconscious: NGT
Conscious: By mouth
Basic part of the device that divides the upper & lower respi tract?
TRACHEOSTOMY CAP
If removed accidentally?
1) Open airway
2) straighten the neck; apply AMBU bag
TRACHEOSTOMY CARE SHOULD BE DONE EVERYDAY!
