How do you manage over anticoagulation with warfarin? INR:

<6 - reduce warfarin dose
6-8 - omit warfarin for 2 days and then reduce dose
> 8 - omit warfarin and give 5-10mg oral vitamin K.
What’s the protocol with warfarin if there was a major bleed eg into the brain?
Minor bleed and INR >5?
- stop warfarin
- give 5-10mg IV vitamin K
- give prothrombin complex (beriplex)
Minor bleed-
Give 1-3mg IV vitamin k.
What’s the treatment for anti psychotic induced acute dystonic crisis?
1) procyclidine 10mg IV
2) diazepam
