Congenital Toxoplasmosis appearance in neonates

A

Classic triad of

1) chorioretinitis
2) hydrocephalus
3) diffuse intracranial calcifications

Also shows the blueberry rash

Diagnosis

  • IgM antibodies + for Toxoplasmosis Gondi
  • also can get PCR for toxoplasmosis DNA to confirm

Treatment = pyrimethamine, sulfadiazine and folinic acid

Congential syphillus appearance in neonates

A

Early (<2 yrs)

  • jaundice and hepatosplenomegaly
  • maculopapular rash
  • excessive “snuffles”/ nasal discharge
  • skeletal abnormalities and lymphadenopathy

Late (>2yrs)

  • frontal bossing
  • Hutchinson teeth
  • interstitial keratitis
  • sensorineural deafness
  • saber shins

Diagnosis
- VDRL and RPR w/ dark-field microscopy on culture

Treatment = pencilin- G

Congential listeriosis apperance

A

Usually presents with granulomatosis infantiseptica (vesicular and pustular skin lesions w/ menigitis and sepsis)

is known to cause spontaneous abortions and premature birth

Diagnosis

  • bacterial culture in cold temps on blood agar (shows B-hemolysis)
  • also CSF fluid will show bacterial cultures for listeriosis

Treatment ampicillin and gentamicin

Congenital VZV apperance

A

Shows:

  • encephalitis
  • cataracts and chorioretinitis
  • pneumonia
  • hypoplastic limbs
  • CNS abnormalities
  • “ hypertrophic scarring” (cicatial lesions)

**most common in 1st and 2nd trimester if infected

Diagnosis = DFE or PCR for VZV DNA
- can also do serology for IgM antibodies to VAV

Treatment = VZIG shots with acyclovir

Parvovirus B19 apperance

A

Presents with

  • severe fetal hydrops
  • aplastic crisis (severe anemia)

Very heavily tied to abortions

Diagnosis

  • PCR for B19 DNA
  • serial US to rule out or in fetal hydrops syndrome

Treatment = intrauterine fetal blood transfusion

Rubella congential apperance

A

Presents with

  • sensorineural deafness
  • cataracts neonatal
  • PDA or pulmonary artery stenosis heart murmurs
  • CNS abnormalities

Shows the classic blueberry muffin rash

Diagnosis

  • serology = IgM antibodies
  • PCR for rubella RNA

Treatment = supportive care and hope they survive

CMV congenital apperance

A

Symptoms

  • sensorineural deafness
  • periventricular calcifications
  • petechiae and purpura (blueberry muffin rash)
  • seizures
  • small weight and size

Diagnosis

  • viral cultures
  • PCR = CMV DNA

Treatment = ganciclovir or valganciclovir

HSV congenital appearance

A

Symptoms

  • premature birth and IUGR
  • vesicular lesions
  • keratoconjuctivits
  • sepsis
  • meningeoencephalitis

Treatment

  • viral culture
  • PCR = HSV DNA

Treatment

  • Acyclovir
  • supportive care
