Infectious bronchioloitis (MAC, TB), hypersensitivity pneumonitis, endobronchial spread of tumour Also consider: RB-ILD, pneumoconioses
Cystic lung disease
Emphysema, LAM, LCH
Also consider pneumocystis, LIP
Lower lobe interstitial disease
UIP, collagen vascular disease, asbestos-related lung disease
Also consider: drug toxicity
Upper lobe interstitial disease
TB, sarcoid, CF
Also consider pneumoconioses, LCH
Hyperlucent lung
top 3: chest wall (mastectomy, Poland), Swyer-James, acute asthma
top 3 don’t miss: airway obtruction, PE, pneumothorax
Anterior mediastinal mass
Lympmhoma, thymic, thyroid, teratoma
Middle mediastinal mass
Lymphadenopathy, vascular (aortic or pulmonary artery aneurysm), congenital cyst (bronchogenic)
Also consider hiatus hernia
Posterior mediastinal mass
Neurogenic tumour, lymphoma, developmental cyst (neuroenteric)
Also consider: extramedullary haematopoiesis, mediastinal haematoma
Chronic air space disease
COP, proteinosis, Carcinoma
Also consider: chronic eosinophilic pneumonia, lipoid pneumonia
Peripheral air space disease
COP, eosinophilic pneumonia, infarct
Also consider contusion, sarcoid
Ground glass
Pulmonary oedema, atypical infection, pulmonary haemorrhage
Also consider: ARDS, proteinosis, vasculitis
Hilar adenopathy
Infection, lymphoma, sarcoid
Also consider: mets, pneumoconiosis
Calcified pleural disease
Asbestos, fibrothorax (prior infection or haemorrhage), iatrogenic (pleurodesis)
Bronchiectasis
Post-infection (bacterial, TB, MAC), CF, ABPA
Also consider: obstructive lesion / mass, ciliary dyskinesia
