privileges more available to those with milder symptoms, less to those w severe symptoms, TE suggests symptoms bypassed by seeking reward, discriminates against severely ill, symptoms prevent accessing this technique, reduced use of TE as not appropriate for all patients

more effective in institutional care, when sent home patients lose structure they had to help manage behaviour, questions appropriateness and effectiveness as high chance of relapse when given independence
