If untreated, increased risk for first trimester spontaneous abortion, stillbirths & neonatal mortality
What is the impact of hyperthyroidism in older adults?
Atypical symptoms
Atrial arrhythmia, HF & osteoporosis
What are the two early warning signs of hyperthyroidism in children?
Deterioration of behavior and school performance in a child who previously did well.
What can be easily missed in adults older than 60 years?
Hyperthyroidism
What is the most common manifestation of hyperthyroidism?
cardiac complications, commonly A-fib, CHF, and angina pectoris
What is the most common cause of hyperthyroidism?
Graves disease
What is the second most common cause of hyperthyroidism?
solitary toxic nodule or multi nodular goiter
What are the lab levels of hyperthyroidism?
Low TSH, Elevated T4 and T3
What are the two thioamides?
Methimazole and Propylthiouracil (PTU)
How is MTZ dosed?
Once daily
How is PTU dosed?
TID
What is the most serious adverse effect of thionamides?
Agranulocytosis
How is Agranulocytosis treated?
It is usually rapidly reversible when the drug is d/c. But broad spectrum antibiotics may be necessary for complicating infections
What drugs can thionamides not be given with?
Other drug producing bone marrow depression
What does amiodarone do with those taking thionamides?
Causes destructive thyroiditis
Which drug is given for myxedema crisis (thyroid storm)?
PTU
What drugs can be given even if lab test level have not come back yet?
Beta-blockers
