levothyroxine (synthroid)

A

T3 and T4 diffuse into the cell nucleus and bind to thyroid receptor proteins to attached DNA
thyroid medicine that replaces a hormone normally produced by your thyroid gland to regulate the body’s energy and metabolism when the gland doesn’t make enough or after surgical removal
too much levothyroxine - too much medication (tachycardia, heat intolerance, tremors, weight loss)
vital signs, esp cardiac status
dose is in micrograms
labs and absent s/s of hypothyroidism
life long treatment 6-8 weeks for full effect
must take at least 30 minutes before breakfast on an empty stomach
take w full glass of water

methimazole

A

blocks the synthesis of thyroid hormone
treatment of graves disease
returns thyroid back to normal state prior to surgery
in addition to radiation treatment of thyroid
too much medication hypothyroidism
agranulocytosis - stop treatment
caution in pts w bone marrow suppression at risk for heart failure
