Thyroid interpretation Flashcards
Dog:
tT4: WRI
Rules out hypothyroidism unless tT4 is falsely increased by T4AA
Dog: tT4: decreased fT4: decreased TSH: increased TgAA: positive
Primary hypothyroidism caused by lymphocytic thyroiditis
Dog: tT4: decreased fT4: decreased TSH: increased TgAA: negative
Primary hypothyroidism caused by thyroid atrophy or possibly end-stage immune thyroiditis
Dog: tT4: decreased fT4: decreased TSH: WRI TgAA: negative
Secondary hypothyroidism caused by pituitary gland dysfunction
Sick euthyroidism, nonthyroidal illness, hypothyroxemia caused by effects of drugs (TMS, prednisolone, phenobarbital)
Dog: tT4: decreased fT4: WRI - increased TSH: WRI - increased TgAA: negative
nonthyroidal illness (sick euthyroidism)
Dog: tT4: WRI - increased fT4: decreased TSH: increased TgAA: positive
Primary hypothyroidism (lymphocytic thyroiditis) with T4AA increasing tT4
Dog: tT4: WRI fT4: WRI TSH: WRI TgAA: positive
Thyroiditis without thyroid dysfunction or false positive (e.g. nonspecific binding)
Dog: tT4: WRI fT4: WRI TSH: increased TgAA: positive
Thyroiditis with compensatory increase in TSH production
Dog: tT4: WRI fT4: WRI TSH: increased TgAA: negative
Potentially responding after withdrawal of suppressive drugs or after a nonthyroidal illness
Cat:
Clinical signs suggest: Hyperthyroidism
tT4: increased
Strong evidence of hyperthyroidism.
A concurrent increased fT4 provides additional evidence but is typically not needed
Cat:
Clinical signs suggest: Hyperthryoidism
tT4: decreased - WRI
fT4: WRI - increased
May be hyperthyroidism and/or nonthyroidal illness; T3 suppression test recommended
Cat:
Clinical signs suggest: WRI
tT4: WRI
fT4:
No evidence of thyroid dysfunction
Cat:
Clinical signs suggest: Euthyroidism
tT4: increased
Could be early hyperthyroidism, normal random variation, or inappropriate RI. Confirmatory increased tT4 or positive T3 suppression test is needed before diagnosing hyperthyroidism
Cat:
Clinical signs suggest: Euthyroidism
tT4: decreased
Probably nonthyroidal illness altering tT4 or fT4
Cat:
Clinical signs suggest: Euthyroidism
tT4: decreased to WRI
fT4: increased
Probably nonthyroidal illness altering tT4 or fT4
