THUMB CMC DISLOCATIONS Flashcards
1
Q
Most common direction for thumb CMC dislocations
A
Dorsal dislocations are most common
2
Q
Mechanism of injury for thumb CMC dislocations
A
Axial load with thumb in flexion
3
Q
Pathoanatomy of thumb CMC dislocations
A
- Dorsoradial ligament must be disrupted for dorsal CMC dislocations
- Beak ligament is often stripped off thumb MC but remains continuous
4
Q
Management of thumb CMC dislocations
A
Closed reduction and assess stability
- If stable (Beak ligament intact) = thumb spica 4-6 wks
- If unstable (Beak ligament torn)
- CR and temporary pinning
- reconstruction of dorsal capsuloligamentous structures and temporary pinning
- better abduction ROM and pinch with reconstruction
