Thrombosis and Embolism Flashcards
What is thrombosis?
Formation of a solid mass within the circulatory system
Why does thrombosis occur?
Virchow’s triad
- abnormalities of vessel wall
- abnormalities of blood flow
- abnormalities of blood components
What abnormalities of vessel walls can occur?
Atheroma
Direct injury
Inflammation
What abnormalities of blood flow can occur?
Stagnation
Turbulence
What abnormalities of blood components can occur?
Smoking - activates factor XII
Post-partum
Post-op
What is the appearance of an arterial thrombus?
Pale
Granular
Lines of Zahn (lines of platelets/fibrin + lines of RBCs)
Lower cell content
What is the appearance of a venous thrombus?
Soft
Gelatinous
Deep red
Higher cell content
What are the potential outcomes for a thrombus?
Lysis Propagation Organisation Recanalisation Embolism
What is lysis?
Complete dissolution of thrombus
Broken down by fibrinolytic system
Blood flow re-established
What is propagation?
Progressive spread of thrombosis
Occurs distally in arteries
Occurs proximally in veins
What is organisation?
Reparative process
Ingrowth of fibroblasts and capillaries
Lumen remains obstructed
What is recanalisation?
Blood flow re-established
- one or more channels formed through thrombus
What is embolism?
Part of thrombus breaks off
What are the effects of an arterial thrombus?
Ischaemia
Infarction
Depends on site and collateral circulation
What are the effects of a venous thrombus?
Congestion
Oedema
Ischaemia
Infarction
