What is the “Pearl”?
Dysfunction in the thoracic region may be responsible for complaints referable to distant and seemingly unrelated areas
What do we inspect?
- Scapular position
- Lateral curves
- A-P curves
- Respiration
- Costal cage asymmetry
Name 5 landmarks their levels
- Vertebrae promninens (C7)
- Angle of Louis (T2) (sternal angle)
- Spine of scapula (T3)
- Inferior angle of scapula (T7)
- Iliac crest (L4)
Give a location for viscerosomatic tissue texture changes?
Rib angles and costovertebral articulations
Tissue texture changes for transverse processes
Anterior/posterior component
Spinous process tissue texture changes
Extended dysfunctions
Rules of 3
a) T1-T3 = same level as spinous
b) T4-T6 = transverse processes 1/2 up from spinous
c) T7-T10 = transverse processes 1 up from spinous
d) T11 = transverse process 1/2 up from spinous
e) T12 = transverse process same level as spinous
Type 1
Rotation/sidebending go to opposite sides and into CONVEXITY
Type 2
Single segment same side (non-neutral)
Type 3
Motion in one segment in any plane affects the rest
Where is type 2 commonly found?
Upper thoracic
What is Kappler’s Corollary?
If head is flexed or extended, which is more restricted?
4 things with Type 1
- Related to short leg mechanics and pelvic sideshift
- Seen in idiopathic scoliosis
- Seen in postural patterns from repetitive activity
- Seen in long standing viscerosomatic reflexes
