ECT
Contraindications
Absolute:
- Raised ICP
Relative:
- Severe HTN and/or aneurysms
- Recent AMI
- Bradyarrhythmias
- Pacemaker
- OP
- MAOI or TCA use within the last 14 days
- Anti-epileptics
- Na valproate/lithium
- Benzodiazepines
ECT
Side Effects
Headache, nausea, myalgia (lasting for the day of treatment)
Cognitive defects - confusion, disorientation, short-term memory loss
Psychoeducation
Indications: everything
Establish patients understanding, educate them on the condition. Indicated in patients with poor compliance
CBT
Indications: anxiety, depression, eating disorders, phobias, chronic schizophrenia
CI: melancholic depression
DBT
Group and individual therapy. Crisis management skills, interpersonal skills, emotional regulation
Indications: borderline PD
Psychodynamic
Reflective. Look at the past (e.g. childhood), interpret it and ‘free’ yourself from it.
Indications: some PDs. Not suitable for most conditions
CI: people with poor self esteem
Psychoanalysis
Patient lies on couch and talks (free association)
Not very useful - only if the patient is intelligent, capable of abstract thinking and willing to cange
Hypnosis
Indications: addiction, anxiety, phobias
Family therapy
Structures discussions. Explore interactions and relationships
Indications: eating disorders, if the child is the patient/symptom bearer
Group therapy
Sharing different experiences of the same disease
CI: eating disorders
