Health and Social care team
the creation of large building housing wide ranges of services being offered w/I primary healthcare premises has created this team- many community based services w/ overlapping interests + responsibilities
GP partner
1st point of contact for most pt. - bulk of work is carried out in consultations/home visits
provide complete spectrum of care w/i local community - dealing w/ problems combining physical, psychological, social components
work w/ teams of other professions + aid pt. to take responsibility for their care
independent contractors to NHS - responsible for providing adequate premises to practice from + employ own staff
Practice nurse
work in surgeries as part of the team - may be several all sharing duties, may be working alone
directly supervise healthcare assistants obtaining blood samples ECGs minor and complex wound management including leg ulcers travel health advice and vaccinations child immunisations and advice family planning & women’s health including cervical smears men’s health screening sexual health services smoking cessation
District nurse
visit people in own homes/residential homes - provide increasingly complex care for pt. + supporting family members
teaching + support role - work w/ pt. to enable them to care for themselves/family members to teach them how to care for relatives + accountable for own pt. caseloads
keep hospital admission and readmission to minimum
assess healthcare needs of pt. + families - monitor quality of care they receive + professionally accountable for care delivery
Midwife
provide care during all stages of pregnancy, labour, early postnatal period
many work in community, provide services in women’s homes, local clinics, children’s centres, GP surgeries
can be wards based - antenatal, labour, postnatal, neonatal wards
Health visitor
lead + deliver child + family heath services (pregnancy - 5yrs)
offering parenting support and advice on family health and minor illnesses
new birth visits which include advice on feeding, weaning and dental health
physical and developmental checks
providing families with specific support on subjects such as post natal depression.
provide ongoing additional services for vulnerable children + families
Being skilledat identifying vulnerable families meanshealth visitors can enable parentsto expresstheir needs and decide on thesupport they receive.The type of support can include:
referring families to specialists, such as speech and language therapists
arranging access to support groups,
organising practical support - for example working with a nursery nurse on the importance of play.
contribute to multidisciplinary services in safeguarding + protecting children
Health visitors are trainedin recognising the risk factors, triggers of concern, and signs of abuse and neglect in children. They also knowwhat needs to be done to protect them.
Often, they arethe first to recognise whetherthe risk of harm to a child hasincreasedto a point where actions needs to be takento protect them. They’ll alsomaintain contact with families while formal safeguarding arrangements are in place, ensuringfamilies receive the best possible support during this time.
As they areinvolved in every stage of the child protection process, including serious case reviews, they are sometimes called upon to appear in court to explain the action taken.
Macmillan nurse
specialise in cancer, palliative care - provide support + care to pt. w/ cancer + their families, from diagnosis onwards
Specialised pain and symptom control
Emotional support both for the patient and their family or carer
Care in a variety of settings – in hospital (both inpatient and outpatient), at home or from a local clinic
Information about cancer treatments and side effects
Advice to other members of the caring team, for example district nurses and Marie Curie nurses
Co-ordinated care between hospital and the patient’s home
Advice on other forms of support, including financial help.
don’t offer routine nursing tasks
Pharmacist
expert in medicines + their use
most = hospital pharmacy, community pharmacy, primary care pharmacy - ensure pt. get max benefit from medicines
advise medical + nursing staff on selecting + appropriate use of medicines
provide info to pt. on how to manage their medicines to ensure optimal rx
able to take additional training to allow them to prescribe medicines for specific conditions
