Preventative level of care
Focus on reducing and controlling risk factors for disease
(Wellness visits, immunization)
Primary level of care(health promotion)
Focus on improved health outcomes and requires collaboration between healthcare providers. Health promotion programs are designed to reduce the incidence of disease.
(well – baby care, family, planning, prenatal care)
Secondary level of care (acute care)
Provided by a specialist or agency, and requires more special life, skill, knowledge or equipment, then primary care can provide
(Urgent care, hospital, emergency room, acute medical – surgical care)
TERTIARY level of care(acute care)
Specialized consultative care, usually on referral from secondary medical personnel that requires care over an extended period of time
(ICU)
Restorative level of care
Go to help individuals regain Maximal functional status, and enhance quality of life through promotion of independence and self-care
(Home care, pulmonary rehabilitation)
Continuing level of care
Provide health, personal and social services over a prolonged period of time
(Assisted living, respite care, adult day care)
Palliative care
For patients and families who are expecting problems related to life-threatening illness
Hospice
Allow patients to live with comfort, independence, and dignity, while using the pain of terminal illness
Pay for value
Pay for value ties reimbursement to quality
Example: if hospital performs, poorly and quality scores, they receive lower payment for services
Patient centered care
Respectful, responsive, talking into account. Individual preferences, needs and values.
What are the eight principles of person centered care?
- Fast access to reliable health or advice
- Effective treatment delivered by trusted professionals.
- Continuity of care and smooth transitions.
- Involvement and support for family and carers
- Clear information, communication, and support for self care.
- Involvement in decisions, and respect for preferences
- Emotional support, empathy, and respect.
