What is the basic patho in testicular torsion
Inadequate fixation of lower pole of
testis to tunica vaginalis
Absent fixation
Testis twist on spermatic cord
Produce ischemia
Causes of testicular torsion
Trauma, vigorous physical activity, or
spontaneous
What are the s/s and clinical findings of testicular torsion
• Acute onset of moderate/severe testicular
pain
• Profound diffuse tenderness &
swelling
• Onset of symptoms several hours
later
• After physical activity or trauma
• Nausea & vomiting
• Abdominal pain
• Asymmetrically high-riding testis
• Hydrocele
• Overlying erythema
Physical Exam
• Negative cremasteric reflex
• Tender mass
• Superior to the testis
Diagnostics of testicular torsion
Mainly through physical exam
US to confirm lack of perfusion
Treatment plan for testicular torsion
Surgical vs non-surgical
Ischemia greater than 8hours can lead to infarction
