What is ballard score
assess newborn age, higher score means more mature, use for first 24hr (posture: observe flexion of limbs out, square window: wrist bend, Arm recoil: rate and strength of arm recoil after straightening arm, popliteal angle: knees to chest and extend leg and measure angle, Scarf sign: cross arm over chest and should go to midline, Heel to ear: heel to ear and leg should go back down
1st period of reactivity
active and alert, HR 120-160, RR rapid and irregular 40-60, startle and cry, active bowels, first meconium, good bonding time
2nd period of reactivity
2-8hr post birth, alert, increased HR, RR, muscle tone, mucus production, suction nose and mouth, meconium passed.
evaporation
water evaporates and takes heat with it, dry baby, bath in warm water
conduction
transfer of heat from baby to cool surface, skin to skin, warm surfaces, use blankets on surfaces
convection
air blowing heat off baby, move baby away from drafts or windows
Radiation
heat lost to colder environment, preheat rooms, away from windows
brown fat
role in thermoregulation, form of heat broken down in body, deplete in several weeks.
Cold stress
with temp decrease, baby becomes hypoglycemic, tachypneic, respiratory acidosis, decreased pulmonary perfusion, pulmonary htn. baby is cold, cyanotic, rr distress, need to rewarm baby.
interventions for thermoregulation
warm baby, use blankets, heat surfaces, monitor temp, dry baby.
Cord care
clamp removed when stump drys in hospital, falls off in 1-3wks, dont peel off, keep clean and dry, sponge bath until falls off, fold diaper under cord
infection s/s of cord
foul smell, drainage, redness, swelling, oozing pus, pink bump near it, oozing blood
circumcision care
needs to be 12hr old and void prior to procedure, no feeding one hour prior, give vit K before, cover penis in vaseline, plastibell, Gomco and Mogen technique
Breastfeeding benefit
contains nutrients and antibodies, protection from diseases, low risk of obesity, reduced risk of SIDs. in moms reduced breast cancer, type 2 dm, weight loss, osteoporosis, emotional benefits
Latch positions
cradle - babys head in crook of elbow, moms arm holds back and butt, abdomens touch
cross cradle - babies head in hand, back on arm, abdomens touch, easiest latch
football hold - baby on side of mom under arm, head supported by hand, most control
side lying - both lay down, caution mom can fall asleep
what can cause bad latch
engorgement, inverted nipples, slippery nipples, breast hold too close to baby, baby with neuron issue, overly agitated
how to get good latch
bring baby to breast, upper and lower lips should flange out with chin and tip of nose press into breast, wide mouth
frequency of feeding
feed on demand, hourly to 8-12x daily, 5-20 min per breast
baby signal of hunger
baby moves hand to mouth and cry
storage of milk in fridge
freshly squeezed for 4 days, thawed 1 day
storage of fresh milk
4 hours, thawed left on counter for 1-2hrs
storage of freezer milk

automated auditory brainstem response - electrodes on forehead, assess electrical activity of brainstem, auditory nerve, cochlea. otoacoustic emissions - ear probe detects responses of hair cells in cochlea
