How does ductus arteriosus change
vasoconstriction from no prostoglandins and increased O2 close shunt
Steps to care for newborn right after birth
- dry baby (keep em warm)
- maintain airway (bulb suction, make them cry)
- APGAR score (less than 8 take to warmer)
- Assessment (head to toe, VS q2hrs, measure)
- Medications (give 1hr post birth)
A in apgar
activity - 2 points spontaneous mvmt all extrems, 1 point arms and legs flexed with little mvmt, 0 points no mvmt
P in apgar
pulse - 2 points HR >100, 1 point <100, 0 points no heartbeat
G in apgar
grimace - 2 points cry or agitated, 1 point mild reaction, 0 point no response
2nd A in apgar
appearance - 2 points no cyanosis, 1 point acrocyanosis, 0 point completely cyanotic or pale
R in apgar
respirations - 2 points cry vigorous, 1 point slow irregular cry, 0 points no respirations
Normal vitals for newborn baby
Pulse 120-160, RR 30-60, temp 36.5-37.2C
Measurements for newborn
head circumference, length, weight, chest circumference
Identification of baby
baby should not leave room before given identification, two bands on baby, one on mom, has alarm when baby leaves exits.
medications to give baby
Vitamin K - 1mg IM injection, prevent bleeding
Hep B vaccine - IM injection, first vaccine
erythromycin ointment - .5% ointment in eyes, prevent gonococcal ophthalmia
Tonic neck reflex
turn babys head quickly to side, one arm and leg should extend on the side, others flex. lasts 3-4 mo, longer means cerebral palsy
Moro reflex
startle, goes away 2 mo, longer means neurodevelopmental problem, exaggerated is neonatal abstinence syndrome, asymmetric means injury to one side
Rooting reflex
when mouth, chin or cheek touched baby turns to that side. goes away 3-12 mo, no response means facial paralysis, neurologic depression
Sucking reflex
finger in mouth and feel for suck, begins in utero, no reflex means neuro depression, NAS, immaturity
Palmar/plantar reflex
grab object or curl toes around object, goes away 3-4 mo
Babinski reflex
tool down foot toes will extend, goes away 12mo, no response means neuron problem
Walking/stepping reflex
when holding baby by torso baby will try to walk, goes away 1 mo.
Acrocyanosis
bluing of extremities, normal 7-10 days PP, central cyanosis concerning
Vernix
thick, white, creamy substance, waterproof barrier and lube for birth, 6hrs leave on normal
Lanugo
fine hair, sheds wk 20 gestation, normal
Congenital dermal melanocytosis
birthmark, flat blue/grey patch, fades away, normal
