TERM PROM Flashcards
What was the aim of the TERM PROM Trial?
To determine whether a practice of inducing labour in women with PROM at term is preferable to a practice of waiting for labour to begin spontaneously if there is no evidence of fetal or maternal compromise
Compared these two approaches and two methods of inducing labour: IV oxytocin and vaginal prostaglandin E2 gel
What was the setting of the TERM PROM trial>
1992-1995
72 hospitals
6 countries
5042 women
What were the inclusion criteria of the TERM PROM trial?
Ruptured membranes
> 37/40
Singleton
Cephalic
What were the exclusion criteria of the TERM PROM Trial?
Active labour
Previous, failed attempt to induce labour
Contra-indication to either IOL or expectant management
What were the intervention groups in the TERM PROM Trial?
- Immediate Oxytocin IOL
- Immediate PG IOL
- Expectant management, then Oxytocin IOL (if not laboured within 4d)
- Expectant management, then PG IOL (if not laboured within 4d)
Expectant management
- either inpatient or outpatient
- checked temp twice daily
- reported fever, change in colour, or odour of amniotic fluid
What was the primary outcome of the TERM PROM Trial
Definite or probably neonatal infection
What was the secondary outcome of the TERM PROM trial?
Need for CS
What were the five main results of the TERM PROM Trial
IOL with OXYTOCIN group were significantly less likely to
- develop Chorioamnionitis (mum)
- develop postpartum fever (mum)
- receive antibiotics (baby)
- stay in NICU for more than 24h (baby)
Rates of CS did not differ between the two groups
What is the conclusion of the TERM PROM trial?
IOL with IV Oxytocin results in a lower risk of maternal infection than expectant management.
Women view IOL more positively than expectant management
How many women were recruited to the TERM PROM trial?
5042
