Describe the surgical procedure for management of marginal tears of the SLBs described by Kadic, Minshall and Wright (EVJ 2019)
What was the reported return to racing following treatment?
A
- GA lateral w affected branch upper, tourniquet at prox MC level
- 5cm proximodistal linear skin incision made at the dorsal margin of the neurovascular bundle
- Underlying metacarpal/metatarsal fascia similarly divided and together with the attached neurovascular bundle separated from the SLB. These were then reflected palmar/plantar sufficiently to expose the palmar/plantar abaxial margin of the SLB
- Torn SLB fibres isolated and removed by sharp dissection and remaining margins of the defect were then debrided with a motorised surgical resector to visibly intact fibrillar arrangement
- Closed in 3 layers - metacarpal fasica, SQ, (simple continuous 3-3.5 metric vicryll) then skin stapled
- Multilayer bandage for GA recovery - maintained w decreasing bulk over 17-32d
- BR 2-4 weeks, then gradually increasing walking 6-8wk

Describe the lesion distribution of marginal tears of the SLBs described by Kadic, Minshall and Wright (EVJ 2019)
A
- 8% FLs, 20.7% Hls, 1 had a FL and a HL injury
- 1% lateral PSBs affected and 18.8% medial
In the forelimbs, 23 lateral and 2 medial branches were affected. In the hindlimbs, three lateral and four medial branches were affected.
Injury to the lateral suspensory branch was most common (79.3%) of which 92% were identified in the forelimbs
Lesions typically affected the abaxial palmar/plantar margin of the SLB
