explanation
-it is also called giant cell arteritis
-collagen disease causing inflammation of extracranial vessels, but can also cause inflammation intracranial vessels ophthalmic artery/post ciliary artery causing optic atrophy and blindness
-vision impaired at some stage, if not treated damage is irreversible
ix
-ESR, CRP>FOLLOWED BY temporal artery biopsy(decide on steroid because long term steroid therapy)
-MRI is also highly specific and sensitive
tx
urgent referral to surgeon for bx and ophthalmologist
-prednisolone 1mg/kg daily for 1/12>should be reviewed after 48hrs
-may take 2-3 years to resolve
-tail off depending on crp and esr values if declining reduce dose>then continue for 1-2 years
-with h2 receptor blocker
