A

-probably due to Iv drug use
-further ix need to be done to check if infection cleared and effect on liver
-20 percent clear spontaneously
-75 percent>chronic hepatitis>5-10 cirrhosis

-indirect coombs: blood immunization needle sharing

-hep infected human blood
-most diagnosed when infection chronic and well established
-risk of liver disease: PCR and LFT
-ALT : 3 test over 6 months>if normal prognosis good no long term adverse liver effects
-In this pt If ALT normal now reassure as it happened 8 years ago>but further ALT monitoring needed
-IF ALT POSITIVE
refer to GI>full liver assessment including bx, antiviral of use if fibrosis present.
liver failure>liver transplant few cases, small risk of HCC

-wait till 6/12 of completing antiviral therapy, ribavirin teratogenic
-mother to baby 5 % RCV positive

counseling risk to others

A

-no risk hugging, kissing, casual contact, sharing food , utensils
-avoid sharing objects with blood contamination: razors, toothbrush
-sexual transmission : extremely low
-need to notify
-give reading materials
