Social worker and client determine why treatment was sought, what precipitated the desire to change now, parameters of the helping relationship (defining roles) and expectations of treatment
Assessment
Client is the source of information of which to define the problem and solutions. Collateral contacts too
Planning
Sw and client develop common understanding of a client’s preferred lifestyle. Develop goals and specific action plans which are agreed upon. Timelines for implementation and review. Smart goals
Intervention
Client must be actively involved in mobilizing their support system. Attention to issues that may threaten goal. Progress is tracked
Evaluation
Subject report of a client. Determine when goals and objectives have been met and whether new goals should be set
Termination
Client reflect on achievements and figure out future supports
