Chancre at site of entry.
Usually three weeks after exposure (10-90 days)
Genitals and anus are usual site
Regional adenopathy is called a bubo
Serologic tests are usually positive
Primary Stage
Secondary syphilis: Usually begins 3-10 weeks after chancre, but can overlap
Many cutaneous eruptions can occur, called syphilids or lues
“Great imitator:” Can look like many other benign rashes
Classically mimics pityriasis rosea
Papules and patches, often oval, may be copper colored, generalized, with involvement of palms and soles
All secondary lesions are highly contagious-use gloves to examine

Secondary Syphilis
Positive serologic test for syphilis without any clinical symptoms or evidence or organism in CSF
Late latent syphilis: Two years beyond initial infection
Latent Syphilis
Up to 60 years after initial infection
Gumma: Small nodule, enlarges and necroses
Develops into punched-out ulcer with gummy base
Ulcerative nodules
Aortic aneurism, CNS signs, dementia
Tertiary Syphilis
