syndrome of inappropriate ADH (SIADH) Flashcards
1
Q
SIADH
A
inappropriately large amounts of ADH
2
Q
what may cause SIADH
A
pos pituitary secreting too much
or
coming from elsewhere e.g. a small cell lung cancer
3
Q
what does excessive AHD result in
A
excessive water reabsorption in CD
water dilutes sodium, resultin in hyponetreamia
urine becomes more concentrated as less water excreted
4
Q
SIADH - what happens to sodium
A
euvolaemic hyponatraemia
5
Q
SIADH - urine characteristis
A
high urine osmolality
high urine sodium
6
Q
symptoms
A
headache
fatigue
muscle aches, cramps
confusion
severe hyponatraemia –> seizures, reduced conscioussness
7
Q
causes
A
post-op infection head injury medications - thiazide diuretics, SSRIs, NSAIDs malignancy esp SCLC meningitis
8
Q
Mx
A
- establish and treat cause
- fluid restriction
- ADH receptor blockers
9
Q
central pontine myelinolysis
A
CPM (osmotic demyelination syndrome) –> complication of severe hyponatraemia being treated too quickly
