hyperaldosteronsim Flashcards
conn’s syndrome
primary hyperaldosteronism
primary hyperaldosteronism
adrenal glands directly responsible for producing too much aldosterone
serum renin will be low as it is suppressed by high BP
primary hyperaldosteronism causes
adrenal adenoma secreting aldosterone
bilateral adrenal hyperplasia
familial
adrenal carcinoma
secondary hyperaldosteronism
excessive renin stimulating adrenal glands to produce more aldosterone
secondary hyperaldosteronism causes
renal artery stenosis
renal artery occlusion
heart failure
renal artery stenosis confirmatory tests
doppler USS
CT angiogram
magnetic resonance angiography
Ix
renin and aldosterone levels BP - htn electrolytes - hypokalamia ABG: alkalosis CT/MRI
aldosterone and renin levels: 1ry hyperaldosteronism
high aldosterone
low renin
aldosterone and renin levels: 2ry
high aldosterone
high renin
Mx
aldosterone antagonists - eplerenone, spironolactone
underlying cause: surgical removal adenoma, renal a. angioplasty
most common cause of 2ry hypertensin
hyperaldosteronism
pt w htn not responding to treatment
consider screening for hyperaldosteronsm with aldosterone and renin levels
