What is additional required for women?
Pelvic exam and pap if over age 21
Baseline mammogram at age 40
Screening for mammogram every 1-2 years between 40-50 years old
Mammogram every year at age 50
What additional information is required for infants and toddlers?
Copy of individualized family service plan (IFSP)
What additional information is required for preschool or school aged children?
Copy of current individualized education program (IEP)
