What is the purpose of EFMP?
Assist service members in providing for the special needs of their EFM before, during, and after relocation required by change of duty assignments

Who qualifies for enrollment?
Family members identified by physicians and/or educational authorities as exhibiting medical or educational disabilities or chronic medical conditions (6 months or longer)

What is the enrollment criteria for EFMP?
Family member with long term or chronic medical, psychological, or educational needs
Enrolled in DEERS
Resides with sponsor (exception for geo-Bach)
How many categories are in the EFMP?
6
What is category 1?
Enrollment for monitoring purposes
What is category 2?
OCONUS and remote assignments may be restricted of support cannot be provided
What is category 3?
Some CONUS and OCONUS limited based on diagnoses and requirements
What is category 4?
Normally, no overseas assignments to include Alaska and Hawaii
What is category 5?
The family member(s) meets the criteria for stabilization to remain in a particular geographic location (homestead)
What is category 6?
Temporary category that requires a stable environment for 6 months to a year for ongoing treatments. Must be updated within 1 year to receive permanent category or disenrollment
Where does the application for EFMP be forwarded to?
Central screening committee (CSC)
What is the purpose of the CSC?
Make recommendations for enrollment packages and forward to Chief, Navy personnel command (CNPC)
What are the locations of major fleet concentration sites?
West coast: San Diego CA
East Coast: Portsmouth VA
What are BUMEDS responsibilities to EFMP?
Maintain CSC/EFM developmental centers
Develop internal program policies for HCP and patient admin to identify and enroll sponsors
Identify EFM coordinator at each Navy MTF
What are the command responsibilities for EFMP?
Appoint EFM POC in local units
Disseminate requirements for mandatory enrollment
Ensure confidentiality
Conduct annual EFM assessments
Conduct annual EFM training
Assure enrollment for FM with special needs
CCC work with sponsors for career progression
What are the MTF coordinator responsibilities for EFMP?
Serve as command POC
Assist staff and sponsors
Provide training
Provide information to sponsors
Provide necessary forms and review completed applications
Forward enrollment package to CSC
Refer families to fleet and family services
Coordinate with SSC
Maintain liaison with BUPERS manger
What are the service members responsibilities for EFMP?
Identity and enroll FM
Review completed application
Update file every 3 years, 12 months before orders, or as needed
Maintain copy of application
What qualifies for disenrollment?
Previous enrolled EFM has recovered
Divorced or legally separated
EFM dies
EFM no longer resides in home, or no longer DEERS eligible
