Sudden Death- Cardiac Flashcards
What is long QT syndrome?
Torsades de Pointes triggered by adrenaline (stress, sport, shock)
What risk increases with severity of QT prolongation?
Sudden death
AD form of long QT syndrome?
Romano-Ward syndrome
AR form of long QT syndrome and what additional feature does it have?
Deafness
Jervell and Lange-Neilsen Syndrome
Diagnosis of long QT syndrome?
QT >480ms on repeat ECG
or genetic mutation known
Management of long QT syndrome?
Avoid drugs (antimicrobials, antidepressants, antiemetics, antipsychotics, methadone) Triggers- strenuous exercise, loud noise
What is short QT syndrome?
Mutation in cardiac K channels
QT <300ms
Presentation and treatment of short QT syndrome?
Young children
ICD implantation
What does Brugada syndrome cause?
ST elevation and RBBB in V1-3
Can become VF/VT
Triggers of Brugada syndrome?
Rest/sleep
Fever
Excessive alcohol
Large meals
What is CPVT?
Catecholaminergic Polymorphic Ventricular Tachycardia
VT/SVT due to adrenaline
Investigation of CPVT?
NORMAL ECG/ECHO
Management of CPVT?
Avoid sports, stress etc.
Beta blockers
ICD implantations
Flecainide
Pathophysiology of Wolff-Parkinson White Syndrome?
Accessory pathways prevent normal delay in AV conduction
Leads to VF
ECG of Wolff-Parkinson White Syndrome?
Short PR interval
Delta wave
