Substance/Medication Induced Depressive Disorder Flashcards
criterion A for substance/medication induced depressive disorder
a prominent and persistent disturbance in mood that predominates in the clinical picture and is characterized by depressed mood or markedly diminished interest or pleasure in all, or almost all, activities
criterion B for substance/medication induced depressive disorder
there is evidence from the history, physical examination or laboratory findings of BOTH (1) and (2):
- the symptoms in criterion A developed during or soon after substance intoxication or withdrawal or after exposure to a medication
- the involved substance/medication is capable of producing the symptoms in criterion A
criterion C for substance/medication induced depressive disorder
the disturbance is not better explained by a depressive disorder that is not sub/med induced
what would suggest a disorder was NOT sub/med induced
“The symptoms preceded the onset of the substance/medication use; the symptoms persist for a substantial period of time (e.g., about 1 month) after the cessation of acute withdrawal or severe intoxication; or there is other evidence suggesting the existence of an independent non-substance/medication-induced depressive disorder (e.g., a history of recurrent non-substance/medication-related episodes).
criterion D for substance/medication induced depressive disorder
disturbance does not occur exclusively during the course of a delirium
criterion E for substance/medication induced depressive disorder
disturbance causes clinically significant distress or impairment in social, occupational or other important areas of functioning
when should the diagnosis of substance/medication induced depressive disorder be made instead of substance intoxication or withdrawal
only when symptoms in criterion A (depression) predominate in the clinical picture and when they are sufficiently severe to warrant clinical attention
list some medications that can induce depressive mood disturbances
stimulants
antidepressants
anticonvulsants
anti-migraine agents (triptans)
antipsychotics
steroids
L-dopa
antibiotics
CNS drugs
dermatological agents
chemotherapeutic drugs
immunological agents
in the case of substance/medication induced depressive disorder, when does the depressive disorder most often have its onset
within the first few weeks or 1 month of use of the substance
once substance is discontinued the depressive symptoms usually remit within days to several weeks–> depends on half life of the substance/medication and the presence of a withdrawal syndrome
when should you consider a different etiology for a suspected substance/medication induced depressive disorder
is the symptoms persist 4 weeks beyond the expected time course of withdrawal of a particular substance/medication
