infarct at border zone between 2 arterial territories
peds stroke RF
SCD
heart disease (intraop)
acute systemic infl: infection, dehydration, IDA, prothrombotic disorders, chronic disease
what is moyamoya and RF
progressive stenosis of distal ICAs with formation of 2o distal collateral vessels
CRT, NF-1, SCD, T21, East Asian
Stroke mgt
Cardioembolic: LMWH/VKA x3mo
Dissection: LMWH/VKA x3mo then ASA x2+y
Neonatal only tx cardioembolic
- LMWH x 3-6mo then ASA if unrepaired cardiac lesion
malignant MCA infarct definition, epi, tx
transtentorial herniation
1-12% in children with MCA-AIS
Tx: decompressive hemicraniectomy
Stroke 2o prevention (non-SCD)
If no dissection or cardioembolic cause: ASA proph
Stroke on ASA, change to clopidogrel or LMWH/VKA
