Q

The Watershed of Venous Drainage

A
  • Azygous vein from thorax runs w/esophagus into left gastric vein (on stomach post to liver)
  • Portal vein = potro-systemic venous anastomosis

Q

Right Crus of diaphragm

A
  • Passage of esophagus from thorax to abdominal area
  • @ the LVL T10
  • Superior to the Aortic/thoracic duct of abdominal aorta @ T12

Q

Parasympathetic Nervous system

A
  • Right vagus runs along with esophagus into the esophageal plexus
  • Plexus @ sphincter of stomach
  • Left vagus runs w/esophagus & OVER aortic arch to rejoin @ plexus
  • Vagus is CN 10 and innervates stomach/esophagus

Q

Symptoms of Achalasia of Cardia (cardiac sphincter) “Sphincter tightly constricted”

A
  • Barium swallow = “birds beak”
  • Food sticks
  • Discomfort post. to sternum=heartburn
  • Regurgitating unmodified food
  • Halitosis
  • Weight loss
  • Hunger

Q

Auerbach’s plexus

A
  • degeneration is linked to achalasia of cardia as cause for constriction
  • Hirschsprung’s is a congenital disorder in plexus that effects LI creating blockage of feces due to NO peristalsis.

16
Q

Symptoms of GERD

A
  • Worse after heavy evening meals
  • Regurgitates metallic taste
  • Mucosal oedema (inflammation=thickening)
  • Most common cause is hiatus hernia
17
Q

Layers of Stomach/Esophagus

A
  • Oesophagus = pseudo stratified squamous
  • Stomach = columnar (glandular) –>crypts/glands
  • Location where esophagus & stomach meet is squamo-columnar junction
18
Q

Oesophago-gastric junction

A
  • Liable due to state of flux
  • Cells less stable
  • Potential tumors/cancer
  • Metaplasia (replacement of one cell type for another)
19
Q

Barrett’s esophagus

A
  • GERD: acid exposure for 5+ years
  • Metaplasia of mucosal layer of lower esophagus to squamous gastric columnar type
  • Pre malignant condition (death)
20
Q

Esophageal cancer

A
  • Fastest growing cancer in western world
  • Due to esophago-gastric junction metaplasia
  • Barrett’s and GERD untreated
21
Q

Characteristics of Stomach

A
  • Gastric juices: HCl, pepsin, intrinsic factor
  • Mixes food with gastric juices = Chyme
  • Protective: bi carb & mucus secretion
  • Hormonal: G cells = Gastrin (precursor to HCl from parietal cells)
  • Epigastric/LUQ
22
Q

Mesentery

A
  • Complex folding of gut requires maintenance of blood supply, innervation, lymphatic drainage.
  • Intraperitoneal: Mesentery double layer of peritoneum BV, nerves, lymphs lie BETWEEN layers
  • Retroperitoneal: Covered but NOT wrapped around peritoneum & BV, lymphs, nerves on surface.
  • Intraperitoneal = mesentary
  • Retroperitoneal = NO mesentary
23
Q

Lesser Omentum/Greater Omentum

A
  • Located superior to stomach (lesser curvature) & inferior to liver
  • Portal triad located here (Bile duct, portal vein, hepatic artery)
  • Greater Omentum: stretches across inferior border of stomach (Greater curvature) blankets the intestines & connects stomach to TC
24
Q

Stomach anatomy

A
  • superior border: Cardia/fundus
  • Middle: Body
  • Inside: Rugae
  • Inferior border: Antrum/pylorus
  • Oblique muscle controls lumen
25
Q

Relations to stomach

A
  • Anterior: Left lobe Liver (URQ) - Ab wall
  • Posterior: Foramen of Winslow (lesser sac) - Pancreas - Aorta, Left Kidney, Spleen
  • Inferior - Great omentum - Transverse Colon
26
Q

Stomach Innervation

A

-Myenteric plexus & Submucosal plexus = Intrinsic innervation - ANS

27
Q

Stomach Innervation (sympathetic/para)

A
  • Symp: Preganglionic T6-T9 –>sympathetic trunk
  • Parasympathetic: Left (anterior)/right (posterior) vagus
  • ALL extrinsic innv.
28
Q

GI tract Pain location

A
  • Foregut = Epigastric
  • Midgut = Umbilical
  • Hindgut = Pelvic
