Stomach & Esophagus Flashcards
1
Q
Boundaries of the esophagus
A
- Superior: Pharygoesophageal junction @ C6 cricopharyngeus muscle (upper esophageal muscle), Cricoid @ C6 (ring of cartilage around trachea)
- Inferior: Stomach @ the esophargogastric junction (T10)
2
Q
Cervical esophagus
A
- Anterior: Trachea & thyroid
- Posterior: Vertebral bodies
- Lateral: Carotid sheath
3
Q
Thoracic esophagus/superior mediastinum
A
- Anterior: Trachea (anterior is aortic arch)
- Posterior: Vertebral column
- Lateral: Parietal pleura surrounding lung
4
Q
Level of T4/5
A
- Bifurcation of right/left bronchus
- Left/Right pulmonary artery
- Arch of the aorta
5
Q
Thoracic esophagus/inferior mediastinum
A
- Anterior: Left bronchus, left atrium of heart
- Posterior: Vertebral column/descending aorta
- Lateral: Aortic arch & thoracic duct-largest lymphatic duct in body (left) & azygous vein (right)
6
Q
Esophageal constrictions
A
- Upper: junction of esophagus w/pharynx
- Upper middle: Aorta
- Lower middle: L. main bronchus
- Lower: @ esophageal hiatus
7
Q
Blood Supply of Esophagus (arterial)
A
- Upper: Inf. Thyroid artery (via subclavian-arch of the aorta)
- Middle: Bronchial artery (via descending aorta)
- Lower: Gastric artery (via descending aorta/celiac trunk)
8
Q
Blood supply of Esophagus (vein)
A
- Upper: Inf. Thyroid veins
- Middle: Azygous vein
- Lower: L. gastric vein
9
Q
The Watershed of Venous Drainage
A
- Azygous vein from thorax runs w/esophagus into left gastric vein (on stomach post to liver)
- Portal vein = potro-systemic venous anastomosis
10
Q
What organs are associated w/portal vein?
A
- Liver (major)
- Stomach
- Spleen
- Large intestine
11
Q
Right Crus of diaphragm
A
- Passage of esophagus from thorax to abdominal area
- @ the LVL T10
- Superior to the Aortic/thoracic duct of abdominal aorta @ T12
- Vagus nerve runs with esophagus @ right crus
12
Q
Parasympathetic Nervous system
A
- Right vagus runs along with esophagus into the esophageal plexus
- Plexus @ sphincter of stomach
- Left vagus runs w/esophagus & OVER aortic arch to rejoin @ plexus
- Vagus is CN 10 and innervates stomach/esophagus
13
Q
Musculature of esophagus
A
- Upper 1/3 = Skeletal/circular
- Middle 1/3 = mixed
- Lower 1/3 = Smooth/longitudinal (peristalsis)
14
Q
Symptoms of Achalasia of Cardia (cardiac sphincter) “Sphincter tightly constricted”
A
- Barium swallow = “birds beak”
- Food sticks
- Discomfort post. to sternum=heartburn
- Regurgitating unmodified food
- Halitosis
- Weight loss
- Hunger
15
Q
Auerbach’s plexus
A
- degeneration is linked to achalasia of cardia as cause for constriction
- Hirschsprung’s is a congenital disorder in plexus that effects LI creating blockage of feces due to NO peristalsis.
