Stomach Disorders: Zollinger-Ellison Syndrome and NSAID Ulcerations Flashcards
What is Zollinger-Ellison Syndrome?
- Tumors of the gastrin-secreting endocrine cells of the pancreas, or less frequently, the duodenal wall or abdominal lymph nodes, resulting in excessive acid production by the GI tract
- The excessive acid causes serious and aggressive peptic ulcers
What are the complications associated with Zollinger-Ellison Syndrome?
- Perforation
- Hemorrhage
- Obstruction
How may Zollinger-Ellison Syndrome cause perforation?
• Ulcers may create holes in the wall of the stomach and or duodenum
How may Zollinger-Ellison Syndrome cause hemorrhage?
• Ulcers may penetrate to an artery, causing internal bleeding
How may Zollinger-Ellison Syndrome cause obstruction?
• Ulcers can cause inflammation that results in an obstruction in the path of food leaving the stomach
What treatment is available for Zollinger-Ellison Syndrome?
- Surgical: Tumor resection
* Pharma: H2R Blockers, Anticholinergics, Antacids
How do NSAIDs cause peptic ulcers?
- The action of NSAIDs encourages mucosal permeability to increase drug absorption.
- In so doing, it also inhibits mucus secretion central to the protection of the stomach lining
- This increases the probability of stomach acids to penetrate the stomach epithelium
- The longer NSAIDs are taken, the more of a compound effect takes place, eventually forming peptic ulcers
NSAID ulcers are commonly occur in those who take NSAIDs for…
• Arthritis
What is misoprostol (Cytotec)?
• A medication is used as a prophylaxis to prevent NSAID induced ulcers
What is the major contraindication of misoprostol (Cytotec) use?
- Not to be given to women who are pregnant as it can cause birth defects, abortion, premature birth or uterine rupture
- It is used as a medication to induce abortion
