What is the manifestation of primary syphilis
there is a chancre at the inoculation site and many bacteria
chancre is hard, firm, cartilaginous almost, it is raised AND it is painless

- rash (on palms and feet soles)
- mucosal lesions
- lymphadenopathy
- fever

- asymptomatic

Diagnostic testing for syphilis
darkfield looks for spirochetes (greater thatn 95% sensitivity for looking at chancres)
Characteristics of the non-treponemal test
It is not great for primary, but very sensitive for secondary and tertiary syphilis. This will wane with time
characteristics of the treponemal test
this is positive for life. not very good at primary, but very good for secondary and tertiary

What is the species of Lyme Disease
B. burgdorferi
What s characteristic of the localized B. burgdorferi infection
starts in days. An erythema migrans is found at a localized area of the skin. This is made by clinical diagnosis
Disseminated B. burgdorferi
spreads over weeks. Found at the skin (multiple EM), CNS (meningitis and neuritis), and heart (arrythmia).
dx: ELISA or western.
