Lack of normal bacterial flora (lactobacilli)
Other “bad bacteria” like Gardnerella vaginalis grow
Fishy odor
BV Diagnosis
Clue cells seen on saline wet mount
Positive “whiff” test with KOH
BV Treatment
Metronidazole PO or vaginal cream
What should you avoid when taking metronidazole?
Alcohol
Pt would vomit if ETOH use
Vulvovaginal Candidiasis (Yeast Infection)

Frequently happens when taking ABX or uncontrolled blood sugar
Thick, curdy discharge, extreme vaginal itching, dysuria

Hyphae seen on KOH wet mount

Vaginal creams or Fluconazole (one time pill) PO
Trichomoniasis
Microscopic protozoan
Yellow-green frothy discharge
Inflammation or vagina and cervix
Trichomoniasis Treatment
Metronidazole 2g PO
(avoid ETOH)
Trichomoniasis Education
Partners should be treated
Avoid intercourse until med completed and symptom-free
Chlamydia
Vaginal burning, frequency of urination, friable cervix (bleeding)
Can progress into PID
Chlamydia Treatment
Azithromycin 1g PO
Chlamydia Education
Partners should be treated
Abstain from intercourse for 7 days
Gonorrhea
Neisseria gonorrhoeae bacterium
Can progress into PID
Asymptomatic usually, but may have greenish yellow discharge, urinary frequency
Gonorrhea Treatment
Ceftriaxone 250mg IM + Azithromycin 1g PO
