A 63 year-old college professor presents with a 2-day history of lower back and right lower limb pain.

• *History:**
• Patient states that pain came on suddenly while pushing a car stuck in a snowdrift.
• He said it felt as if “something had snapped” in the lower part of his spine.
• NSAIDs provided no relief.
• The pain extended down the posterior aspect of his right thigh and leg.
• He also noticed some numbness and tingling over the lateral part of his right leg, foot and little toe.
