High intensity statin
Atorvastatin 40-80 mg
Rosuvastatin 20-40 mg
Moderate intensity statin
Rosuvastatin 5- 10 mg Lovastatin 40 mg Simvastatin 20-40 mg Pravastatin 40- 80 mg Atorvastatin 10-20 mg
Low intensity Statin
Lovastatin 20 mg
Pravastatin 10-20 mg

GI upset

Rhabdomyolysis

Statin + ___ increases risk of rhabdomyolysis
Fibrate
Start on low doses of statin if
pt on 3A4 inhibitor renal insufficiency (exception: atorvastatin)
Contraindication with simvastatin
Intraconazole, Ketoconazole, Posaconazole
Erythromycin, Clarithromyscin, telithromycin
Do not exceed 10 mg Simvastatin with
Amiodarone
Verapamil
Diltiazem
Do not exceed 20 mg simvastatin with
Amlodipine
Pravastatin is metabolized by
CYP450
Characteristics predisposing pt to statin SE
multiple/serious comorbidities
history of previous statin intolerance or muscle disorders
unexplained ATL elevations >3x normal
>75 years of age
Rhabdomyolysis
condition resulting in muscle cell breakdown
can lead to renal failure or other organ failure
Signs of Rhabdomyolysis
Muscle pain, tenderness, weakness
Dark urine, fever, malaise, N/V
Elevated CK
