A

  • No impairment/No cognitive decline
  • No obvious signs of dementia
  • individuals able to function independently

A

Very Mild Cognitive Decline
-Dementia signs are barely noticeable 
-Appears as forgetfulness associated with aging
-Not evident to family or physician

Describe stage 3 of BCRS

A
No dementia present
Mild Cognitive Decline
Pts able to do most basic ADL's
Symptoms:
-Some forgetfulness and memory loss
-increased repetition of tasks (sense something isn't right but not able to figure it out)
-Loss of concentration
-confusion while driving
-trouble managing medications
-misplacing items and unable to retrace steps
-Difficulty managing finances
-May need occasional help with IADL

A

Early stage of Dementia
Moderate Cognitive Decline
-Pts have “trouble doing routine tasks”
Symptoms:
-incontinence
-difficulty word finding
-increase in memory loss and forgetfulness
-inability to use or find the right words and phrases
-Difficulty with challenging mental math exercises
-increase in social withdrawal

A

Mid-stage of Dementia
Moderately Severe Cognitive Decline
-Pts need some assistance with their daily activities
Symptoms:
-Significant memory loss with inability to recall home address, phone number, or personal details
-Need help selecting appropriate clothing for the climate, season, or occasion
-Confusion about location or chain of events
-Difficulty with simple mental math exercises

Describe stage 6 of the BCRS

A

Mid-stage of Dementia
Severe Cognitive Decline
Requires significant help with day to day activites
Symptoms:
-Requiring help with: dressing, toileting, wiping, flushing
-Wandering and getting lost if not supervised
-Inability to recall the names of familiar persons but able to recognize familiar faces
-Sleep disturbances
-Changes in personality or behavior (paranoia and hallucinations)

-loss of language (unable to speak and communicate)
