Preparation
Preparing to change, taking first steps, setting goals
Writing out an action plan and share it with others
Action
Committing time and energy to changing.
Perform the new behavior, monitors and manages results
Maintenance
Striving to integrate new behavior permanently.
Persevere through struggles. Also helping others along your own path
Adoption/Moving On
New behavior is routine.
After permanent change, move on to other goals
