Define Cauda equina
Compression of the terminal nerve roots
Symptoms of cauda equina
Pain (bilateral burning radiating down the leg), Urinary retention, Faecal incontinence
Signs of cauda equina
Palpable bladder, Hyporeflexia, Bilateral leg weakness, Bilateral leg paraesthesia, Decreased anal tone
Investigations for cauda equina
MRI
Management of cauda equina
Decompression within 48hrs
Most common site of disc herniation
L5/S1
Symptoms of disc herniation
Pain (in the lower back radiating into the leg, worse on lifting or standing)
Signs of vertebral disc herniation
Weakness, Positive straight leg raise test, Trendelenburg gait
Describe the straight leg raise test
Passive raising of the leg causes pain, relieved by flexion of the knee
Investigation for disc herniation
MRI
Management for disc herniation
Conservative: Physiotherapy, Rest
Medical: Pain ladder, Steroid injection
Surgical: Discectomy, Laminectomy
Causes of back pain
Degeneration, Malignancy, TB, trauma
Common tumour origins for bony metasteses
Lung, Kidney, Breast, Prostate, Thyroid
Red flag symptoms for back pain
Thoracic pain, Worse at night, Neurology, Systemic features, Past cancer, Recent infeection, Steroids, IVDU, Younger than 18, Older than 55
Investigations for back pain
X-ray, MRI
